Preventive strategies for medication-related harm in healthcare

Authors: Rita Martins

Medication-related harm remains a leading cause of preventable patient harm, particularly in older adults with polypharmacy. This article highlights the importance of integrating patient experiences, evidence-based strategies, and system-wide safety cultures to reduce harm. Practical approaches include patient-centered medication reviews, pharmacist-led counselling, and the use of real-world safety data to inform improvements. Emerging tools such as generative AI offer potential to enhance pharmacovigilance and decision support, though their safe use depends on rigorous validation and regulation. Ultimately, safer care requires shared responsibility, with technology serving to strengthen, not replace, human compassion and clinical judgement.
